Open Gym

Open Gym requirements - Each participant needs to have a completed waiver form along with Code of Conduct to use Open Gym. Once completed, an ID card will be created for you. This card will be required for all future check-ins. All participants 14 and under must have a parent/legal guardian present while using Open Gym.

Lake Forest Residents and Commercial Residents
Fee: Free. There is no charge for initial issuing of the ID Card to all Lake Forest residents and commercial residents. Replacement cards are $15. Lake Forest Residents must show proof of residency:

  • Driver's License, California ID or Utility Bill showing a Lake Forest address.
  • Minors may provide an AERIES printout from the Saddleback Unified School District showing a Lake Forest address.
  • The waiver form must be filled out (please confirm information).

Commercial Residents must show proof of Commercial Residency:

  • A "Commercial Resident" is considered individuals who live in Lake Forest.
  • A "recent" pay stub, business card, or an official letter on company letterhead from the Human Resources Department within three (3) months from the date of application showing a Lake Forest address.
  • P.O. Boxes may prove residency by providing one utility bill in their name, showing a service address in Lake Forest.
  • The waiver form must be filled out with their personal information.

Non-Resident Fees

  • $5.00 daily fee
  • $100 per the calendar year (begins January 1 to December 31 of the following year)

*Group 2 rates – If gymnasium is utilized outside of center hours, an additional $12.00 per hour rate applies.

All gymnasium fees will be billed on a monthly basis. Payments must be made by check or cash to the City of Lake Forest. Payments will be considered delinquent 30 days after the invoice is issued. Organizations submitting late payments will jeopardize their ability to use City of Lake Forest facilities in the future.


To reserve the basketball court or volleyball court inside the gymnasium, a permit is required. A Group User Information Packet, forms, and applications can be printed under Applications and Forms. To participate in the field allocation process, a team or league must complete the following paperwork:

  • A team or league information card;
  • A field reservation form, noting preferred practice or game times or requesting use of the batting cages or gymnasium.
  • Valid proof of insurance, with an endorsement certificate, conforming to the City's required coverage limits; $1 million dollar policy listing the City of Lake Forest as additionally insured.
  • All paperwork must be submitted by established deadlines.

An application can be denied if:

  • Requested amenities are not available;
  • The requested use is inconsistent with applicable laws/rules; or,
  • The requested use could be detrimental to the best interest of the City, as determined by the Director of Community Services.

Gymnasium reservations that are unused are subject to a "no show" penalty as follows:

 1st offense: Written warning
 2nd offense: $20.00 per penalty
 3rd offense: $35.00 per penalty and potential loss of future facility time

Each applicant shall be at least 21 years of age and act or designate one person to act as the coordinator of all rental matters.

Proof of Residency (Group 2, 3, and 4)

Acceptable proof of residency must be submitted for all Lake Forest Residents. Residents of Foothill Ranch, Portola Hills and parts of Trabuco Canyon (92610, 92679 and 92630) will be recognized as Lake Forest residents. Proof of Residency includes a recent copy (within the past 3 months) of utility bill, SVUSD Aeries Portal printout, or a California Driver’s License with a Lake Forest address. The Community Services Department reserves the right to request additional documentation, if the need arises. No league will be designated field allocations without proof of residency for all rosters. Leagues submitting fraudulent rosters are subject to the loss of all allocated time.
